Currently, all I have at my hospital is a list of patients from pharmacy on vesicants. I would like to implement scoring system similar to the braden that would indicate for further assessment for midline,cvc, or picc. Bard has given me a great template for scoring for vascular access needs. I am wondering if anyone has implemented this as part of there admission assessment charting?
