2016 INS Standards address this in Phlebotomy standard. All samples must be labeled at the time the sample is taken and in the presence of the patient. Preanalytic phase is the most common time when errors occur. So it only makes sense for the person obtaining the sample to do the correct patient identification and sample labeling. Lynn
