What is the standard for infusing vanco via midline?

There is no set standard to infuse or not to infuse, but there never really has been such a standard. Vancomycin pH is always going to be low, but that is only one factor of infusate characteristics that must be assessed. Other factors include the planned length of therapy, number of compressible veins in both extremities, other patient risk factors for thrombosis. So it depends on many factors. The standard would be that the inserter must assess all of these factors. Lynn
