Can anyone share policy language regarding tip location for CVADs in policy? Currently we are engaged in problems related to suboptimal tip location of implanted ports. I have been asked by the CNO to ask around for policy language related to this. We have had several upper extreme SVC or brachiocephalic port catheters fail to return blood. We have yet had physicians change practice. What we can do is write nursing policy and stand by our policy related to evidence. This is where we need help.
If you have policy langage related to tip location could you respond with the name of your hospital and quote your policy? I have heard that some hospitals consider upper SVC for PICCs malpositioned and will exchange them.

You could use language similar to what is in the INS Standards. "CVAD tip location with the greatest safety profile is the cavoatrial junction for adults and children. Tip location within 2 cm above or below this location are acceptable." Lynn
