A physician I once worked for recently ask If I would be interested in starting PICC lines in his clinic on an as needed basis. I am considering this strongly. Should I incorporate (LLC)? Any suggestions on what else I might need. I can see this developing into bigger things.
The chosen business structure is a decision based on taxes. I operated as a sole proprietor for about 5 years before I incorporated as an S corp. So talk to you accountant. You will definitely a business license and malpractice insurance as a self-employed nurse, which is more than the policy as an employee. There is so much more to think about an not enough time or space to write it here. In the 2nd edition of the INS textbook there is a chapter on entrepreneurial roles that might be helpful to you.

You will need a medical director, other requirements might be state specific. What state will you be in?
Why do you need a medical director? As nurses we practice under our own license. We are also following the orders of the physicians for each specific patient. We know much more about the practice of PICC insertion and infusion therapy than the vast majority of physicians. I am curious as to why a medical director would be required.
