The following is from a P&P I am reviewing. I understand the smallest catheter in the largest vein will decrease risks but how much is using a Y set to infuse an isotonic solution really going to make. Lynn, I hope you can explain and tell me if I am correct in adding "extravisation" along with the already documented phlebitis? Somethimes I put to much thought in a subject and need feedback.

"Hypertonic saline solutions of 3% or less should be infused through a central line to decrease the risk of phlebitis.
3.1.4.2 When a central line is not available the hypertonic saline solution may be infused through a peripheral line. The hypertonic saline solution is to be Y-sited to an equal infusion rate of an isotonic saline solution."
