I am in the process of putting together documentation records for PICC lines that will be placed in an Out-patient Imaging Center.
Although I have created documents in the past working within a hospital setting, I am looking to update and create something geared
more for an outpatient. I am seeking help from all of you, my vascular access colleagues, as it would be helpful to have other
documents to reference when creating new ones especially for a setting like this. Even if you place PICC's for OP's in a hospital
setting and don't use separate documents, I would very much be interested to see what documentation you are using for them.
The following are the documents I am seeking:
1. PICC Insertion Record detailing pre-assessment, intra-procedure and post-procedure events.
2. PICC line consent and policy geared specifically for an OP center.
3. Discharge Informational/instruction guide for the patient
4. QI indicator record specific for an OP.
