There are numerous ways to format a reference and it all depends on the format used by the publication. Anytime you make a direct quote from any reference, add the page number for that text to the end of the bibliography information. Here is the APA 6th edition method but it is not the only method:
INS. (2011). Infusion Nursing Standards of Practice. Journal of Infusion Nursing, 34(1S).
Here is another:
