Pediatric CVCs: Saline flush or saline/heparin flush

Could you please let me know about for your practice related pediatric patients and CVCs?   

Do you use saline only for CVC flushes?  Any problems with the transition to saline only?  Any literature that supports this in pediatric patients?  Willing to share your success stories with me?

Do you use saline and heparin for all CVC flushes?  Any literature that supports continued use of heparin for pediatric patients?
