Need advise asap please!

Hello everyone. We have a young patient with an IJ whose current TSM is not adhering to his skin. The nurse tells me she uses skin protectant and is applying the SorbaView brand TSM dressing correctly, but needs to change the dressing at least twice weekly and has problems with it in the meantime. Short of making every 48 hour visits to this home patient, does anyone have suggestions for a specific TSM product? This patient has extremely fragile skin with eczema

I have been using OpSite IV3000 for years.  I 'wear tested' it on myself and it outlasted Tegaderm brand.  I wore it outside running in the summer S. IN heat/humidity and it stayed on when the Tegaderm came off in less than 24hrs.  We get very few IJ's so can't speak to that.  Maybe the company would get you some samples.  we also have fewer skin irritations with this than we did the Tegaderm version.  And when patients come back into the hospital, they tell them they have to have the IV3000....LOL.  Good luck.

Coverderm and Sorbaview are two other brands I have used (with success) on problem skin.....Not pushing the product..Just the good outcome!

You also want to make sure the patient's head is turned away and up as far as possible when applying a dressing to the jugular area.  This helps it from tugging and coming loose with head movement.

 I don't know the technique used for dressing this, but when dressing a jugular line I have always curved the external part of the catheter like an up-side-down "J" and had the catheter exit at the bottom of the dressing.  I also did all the things previously mentioned - skin prep, turning head strongly away while applying the dressing, etc, and making sure any skin prep product is totally dry before applying the dressing.  This has worked very well, however, if the skin is moist and broken, or patient is sweating a lot, more frequent dressing changes may be necessary.
