We have a patient with a colonized Implanted Port. She is colonized with staph epi. I know it is possible to pack the port with antibiotics but I can't find any protocol for it. The plan is to use Vancomycin.  Does anyone who does this want to share their protocol? I would really appreciate it. You can post it here or send it to my email address [email protected]. Thanks in advance.
Vanco 2.5mg/ml with Heparin (usually 10u/ml) for Methacillin Resistant bacteria.
Alternate: 100 U Hep 50 mcg Vanco/ml
Or for Methacillin Suseptable bacteria you can use Cefazolin 5mg/ml with heparin.
