Working on a process to address new equipment that is introduced to facility that meets JCAHO tubing reconcilliation requirements, ie: Â A. Â evaluation for possible misconnection errors, B. Â No use of leur-type sryinges for oral or enteral medication administration. Â Would you be willing to share your process? Â Also, if you have a form that assesses the risk of misconnection for any new product being piloted that you would share???? Â Any information is appreciated.
