IV Fat Emulsion and filtration

 I am seeking feedback about how you infuse IV fat emulsion, primarily from hospital nurses. Do you see 3-in-1 admixture used in the hospital or are you still piggybacking the IVFE into the primary PN line? If piggybacking, are you attaching it below the 0.22 micron filter or do you have a 1.2 micron filter in the IVFE line? Thanks for your comments. Lynn

 My husband works on a cardiac stepdown unit, I asked him.  He never sees 3-in-1.  He hangs PPN and TPN regularly, on a dedicated line with a .22 micron filter and hangs the lipid between the filter and the vad.  They say "never filter fat" to remember where to put the filter.  In his hospital lipids hang once a day for about 12 hours. (edited, I said 6 hours in error)

We hang lipids once nightly

We hang lipids once nightly over 12 hours from 2100-0900 and piggy-back them in to the lowest injection port of the TPN line.  No filter.

Because of the high nutrient

Because of the high nutrient composition and the number of additives use to formulate TPN it is recommended to filter with a 0.22 micron filter which is a bacterial filter as well as particulate.  If using a TPN solution with lipids, a 1.2micron should be used. Yes, it will not filter out the smaller particulates but lipids will clog a 0.22 filter.

 Thanks Tom, but my question is different. When IVFE is given by piggyback - not a 3-in-1 admixture - do you add a 1.2 filter for the IVFE line and piggyback the IVFE below the 0.22 filter? Thanks Lynn
