I think this was posted on

I think this was posted on another discussion page but for the answer here, my advise is a complete assessment. Original message does not contain enough details. This infusion requires at least 24 hours each time. So the VAD depends on where the patient will be located. Outpaitnet infusion centers will start the infusion and discharge the patient for the remaining hours. So a PICC will be safer. Who will be disconnecting the patient? Can they learn how to do that safely? If a family member will be doing this disconnection or PIV removal, they must have the proper chemo gloves, gowns, etc. Also USP 800 will require use of closed system drug transfer devices.

Thanks Lynn. All good info. I

Thanks Lynn. All good info. I was mainly curios to the med properties as to which device is most appropriate. The ph and properties I have found seem to be bordeline and PIV was looking to be an option (assuming it was under nursing care during administration). Definitely considering all other factors in decision. ------ update... ultimately the port that was suspected for cause of infection may not be the issue... endocarditis just diagnoses. The time to positivy results held true on this one. Thanks again.

The 46 hr 5FU infusion is

The 46 hr 5FU infusion is tough to run via peripheral, even in inpt.  Most of our FOLFOX are outpt so a PICC or usually a port is used.  We have done a few in the inpt arena and even a couple via peripheral.  5FU will cause tissue to change if it extravasates . . . gets a dry desquamation and dark discoloration about the skin noticeable at 1 month post-extrav.  If peripheral, the nurses have to be very vigilant and the pt's need to be well educated.  But my experience is that it is dicey.
