Our oncology floor has no

Our oncology floor has no specific protocol for chemo extravasations except

Stop the infusion, elevate the extremity, and call pharmacy.  When I spoke to

pharmacy, they said they look it up at the time the nurse calls them.

  Since we(I.V. nurses)are the ones who give peripheral vesicant chemo, we

feel the need to have a better "plan".  One of our nurses located a book in our

library that looks very good and just what we need.  It is

Mader et al., Extravasation of Cytotoxic Agents.c Springer-Verlag Wien 2003 and was

published in Austria.

  A few of the drugs, specifically Paclitaxel, Vinblastine, Vincristine, Vindesine, and Vinorelbine call for hyaluronidase subcu and heat as treatment...many others have

no specific treatment, but some others including Cisplatin, Dactinomycin, Daunorubicin,

Doxorubicin, Mitomycin C  show topical DMSO /cooling as treatment for extravasation.

  In talking with our pharmacy, I've run into a brick wall...when I mention DMSO,

they act as if I'm practicing voodoo!  They tell me it is not FDA approved and the

use of it for chemo extravasation would have to be approved by our Physician &

Therapeutics committee...and I'd better have some excellent references if I expect

to have its use accepted.

  Are any of you using it or know of any good reference articles related to its

use?  Thanks
