Adult clinically indicated peripheral IV resiting

We are in the process of updating our adult peripheral IV resiting policy from every 96 hours to only when clinically indicated.  Does anyone use an antimicrobial disc at the IV site for adult peripheral IVs?  How often do you change the luer lock cap such as a Curos cap if the IV is in longer than 96 hours? How often do you change the dressing if it is dry and intact? Do you change the IV tubing if the IV is not resited within 96 hours.  Thanks so much for feedback.

Some hospitals have gone to the CHG disc on PIV site but that practice is not supported by evidence and there are now TSM dressing with CHG impregnated in the whole dressing. Disinfection caps like Curos must be changed each time they are removed from any site, both PIV and CVAD. They are ONLY a single use device and once it is removed it MUST be discarded. The needleless connector must be changed when the continuous administration set is changed usually every 96 h or at least every 7 days. The PIV dressing should be changed every 5 to 7 days. Every time a PIV or CVAd is changed, the new catheter must be attached to a new administration set. This are all including in the INS Standards of Practiced for 2016.

Thank you Lynn. Do you know which INS standards speak to to these issues. I have Standard 44 but it does not speak to the dressing or cap change.  I can interlibrary loan the appliable standard.
