We have been using the Provena 3cg piccs in our pediatric patients for about 2 years now, but we have seen several instances of the stylet either breaking off or becoming damaged almost to the point of breaking. Has anyone else had this problem?
Any part of an insertion tray that is breaking is a reportable event. Ask your company/hospital that is supplying the product and ask to file a product defect. write up the incident including all the identifying numbers. take pics and send to company. this should be pulled off the market until product is re-evaluated for proper safe manufacturing. this has High potential for a sentinel event. Do not use them.
