initial central line dressing change
When do most facilities change the initial dressing on a newly placed central line?  24 hrs? 72hrs?

There is a movement across

There is a movement across the country to move away from the gauze under the dressing and keep the initial dressing on 7 days or until soiled or starts to come up.  The best dressing the patient gets is while the line is being placed under sterile conditions.  If you hold pressure at that time to control bleeding, then place a Biopatch or Tegaderm CHG dressing, there is little bleeding post insertion which would allow for skipping the gauze and 24 hr dressing change.  Of course, if there is bleeding at the site that saturates either the Biopatch or TegadermCHG, then the dressing would need to be changed.  But better to have a case by case basis than an blanket policy.  This saves time and money.

Please refer to the BIOPATCH web site, www.stopcrbsi.com,  for information on use in the presence of blood, sera and proteins.  I'd also recommend researching the FDA-cleared product inserts for both BIOPATCH and Tegaderm CHG to be clear on their indications for use before using either product.
