OK I e-mailed a bunch.

OK I e-mailed a bunch. This form is filled out with each PICC insertion. We also staple a copy of the insertion record to it. The form comes back to the office and gets put in a binder until PICC is discontinued or pt goes elsewhere with the PICC. Any IV nurse caring for the PICC or any complications makes a note on the form. That way we have a running history of the PICC and any problems at our fingertips. At the end of the mo we review and total all complications. We especially can total the total number of PICCs and daysto calculate the infection rate per 1000 cath days If every team member cooperatesit is easy. Any PICC that is cultured or blood cultures from PICC the data sheet is pulled from the book and placed on a cilpboard for follow-up. It is really easy to figure out at the end of the mo. I am sure there are programs out there to do this with the computer but we were not allowed to purchase anr. PS tried to e-mail katherine evertsz no luck check to see if dots etc in the right place and all spelling correct I have 2 versions I will send along both
