Blood transfusion & pateint safety checks

Do most of your facilities require that two nurses check blood at the patients bedside at initiation of blood transfusion or do they permit a nursing assistant to do this with an RN?

We require 2 Nurses to check blood before giving the transfusion. One must be an RN. I was wondering how many of you transfuse blood through your IV pump?

We transfuse all PRBC's and

We transfuse all PRBC's and FFP on pumps, we infuse over 100 unit a month on our Oncolog unit.

We require 2 licensed nurses to verify each unit. Most of our transfusions are infused via our b/braun outlook pump.

we still are giving blood transfusion the old way, no pumps, but it is required that 2 RN's check at the bed side, always.

We have 2 RN's check the blood band and identification band at the bedside..always.  A tech may pick up blood from the blood bank but it is the RN's resposibility to check the blood and blood band, ect at the bedside.
