Do I always need to place a new PIV proximal to an old AVF/AVG site? This is my facility's policy. What do you think? I never heard this practice before. Please advise. Thank you.
INS SOP references nephrology nurses on using short PIVCs with fistulas and grafts. Use only hand veins on same extremity as a fistula or graft. SOP does not address nonfunctioning fistulas and grafts, however the circulation has radically changed near those old sites. Look at what is planned for the old site. Or are there other sites for a new fistula or graft on the same extremity. So I would stick to using on hand veins.
Thank you for sharing your expertise. So, we don't really have any literature or evidence based to support if we need to place a piv proximal or distal the old AV sites, when we desperate to locate any viable veins in hand, is it? Thanks so much again, Lynn :)

None that I know of. Your judgement after assessment.
