Hello, has anyone had an issue with debris in the end of the IV tubing right near the needleless connector? This is the first time I have heard of this. I did not see the original item and unable to see exactly what they saw in the photos but it was reported as "greenish debris" in the tubing. We are contacting the manufacturer but in the meantime I am asking if anyone else had experienced this with their connector.

This sounds like it might be a drug precipitate due to contact between 2 or more incompatible drugs. Check on the practices of flushing with saline between each and every drug.

That is a good thought - we actually found the problem: user issue, since corrected, and risk reduced/removed house-wide.
