Hi. I recently heard a coment regarding the recommendation of inserting Midlines FR 3 rather than FR 4 to prevent the complication of thrombus formation. Do we have the literature to support it? I have been trying to get some information, but so far all my efforts have not been succesful. Any help is greatly appreciated.
Look for catheter to vein ratio. The catheter should consume no more than a third of the vein diameter. Measure witth your US. Smaller is always better because it decreases contact between catheter and vein wall causing mechanical irritation. Lack of blood flow around a large catheter causes chemical irritation. Both causes disrupt endothelial cells lining the vein wall and starts thrombus development.

Thanks so much for your reply. I did try that, but so far, all I can see is for central line. I know it applies, but I was wondering if there are any articles specifically recommending this for Midlines. Again, thanks for your reply and I will continue my search.
Much appreciated,

There are a few studies on midlines that include catheter to vein ratio. My online CE class on midline catheters offers downloadable evidence tables of all studies on midlines found with a thorough literature search. Find more information at www.hadawayassociates.com
