Medications that are ok for midlines

Re: Midlines, I know the Ins standards changed to osmo <900 for midlines, but it seems like there are a lot of other factors to consider, for example Vancomycin osmo only 300 but it has vessicant properties so would not be appropriate. Can anyone share what their hospital policy is?

The INS Standard on short

The INS Standard on short peripherals also includes no parenteral nutrition and continuous vesicants. Since a midline is in peripheral veins, that would also apply. There are numerous factors involved in this decision. This will be part of a presentation I am giving at an exhibitor theater at INS this year.
