The answer lies in your assessment of the patient and other factors. I am assuming you are referring to the changes that removed the pH parameters from the INS Standards. There is no evidence that those parameters of below 5 and above 9 are the magic numbers. These numbers were added to the Standards in the late 1990's based on a theory and subsequent accumulated evidence did not support those numbers. The Standards provides the criteria to use to make this determination - length of therapy, all infusate characteristics (acyclovir is known to cause thrombophlebitis), osmolarity depends on how your pharmacy mixes any drug. Also number and condition of patient veins and where he will be located for therapy - hospital, home, etc.

How do you feel about infusing Acyclovir through a power midline for 7-10 days

Acyclovir has high rates of thrombophlebitis, so you will probably not reach end of therapy with a single midline. How many compressible veins does patient have? USG long PIV with a third CRV might get half of course, then another might be needed. But I have not looked for any published data on this.
