Not sure what you mean by a "PFO filter". If you are referring to an inline filter on the administration set, that is for the purpose of removing all air bubbles during infusion. To instill tPA, yes, the filter and all administration set, extension set, etc. must be removed and connect the tPA syringe directly to the catheter hub. You would never be able to get the tPA to the occlusion with any length of tubing attached.

I figured that I would need to remove the filter because the tPA would never reach the top of the PICC.
