Lynn Hadaway Associates, Inc. is considering a new form of online learning and would like your comments and feedback on this idea. This would be an online journal club. I would identify clinical issues based on frequent questions I receive and statements in the INS Standards of Practice that are associated with the lowest level of evidence. I would then do a brief literature search for the most recent and appropirate studies pertaining to the identified issue or question. Participants would be expected to know the INS standards of practice about the specific topic as a baseline, be provided with the article(s) to be discussed, and participate in an online discussion. We are hoping to use an online form of videoconferencing so pariticpants can see and talk to each other during the discussion. We would offer continuing education for this. I am thinking each club meeting would be about 90 minutes to allow sufficient time for review, critique and discussion of the studies chosen. This would be planned at the same day and time on a quarterly basis to start and possibly moving to a monthly schedule. I can not tell what the fee for this would be yet because I am not certain of all the costs involved. But I wanted to get your thoughts on this idea before we invested more time and resources into this idea. Thanks, Lynn

Sounds like a fantastic idea! I am definitely interested in participating. I would like to discuss the use of a dedicated vascular access team to decrease CLABSI, drawing blood cultures peripherally vs through a CVAD for CLABSI monitoring, and developing protocols for when to wait on PICC insertions for patients showing signs of sepsis without final blood culture results.

I think this is a great idea. Count me in. I will ask my daughter what was involved in the journal club she was involved in when she was doing her pharmacy residency and if she is still involved. That may give you some insite. She did a journal review for Pradaxa that was very good.

Great idea, A few of us were discussing a similiar concept for our hospital. A few topics of interest would include impact/risk factors of routine lab draws via central lines, the use of ultrasound guided peripheral cannulation, the role of simulation centers in IV Therapy/Vascular Access ( are outcomes improved). Integration of Benner's Novice to Expert in vascular access training programs. How to promote critical thinking...advancing the practice in vascular access/IV therapy.

Thanks to everyone for the ideas. We are moving forward with this idea, although I am still not able to tell you when our first club meeting will be available. We had planned to upgrade our online learning system. Doing this type of synchronous meeting that allows for interactivity will require another type of technology to be integrated with the learning management system. We are working on it. I feel so very frustrated with what I see happening in our hospitals and what I know to be best practice. Talking with fellow IV/PICC nurses about best practice is like singing to the choir, talking with nurses on the floor is like yelling at a brick wall. There must be a way to move forward, to change common practice to best practice. If anyone has ideas I would sure like to hear them.

Wonderful idea and from all the responses, it looks like it will do well. I would like to discuss the issue of a dvt in the arm with a PICC. Should it be removed or not removed and given anticoagulants. I have a vascular physician and a critical care physician with two different opinion.

We have discovered an obstacle in the available technology for our Online Journal Club. I need some feedback from anyone interested in this service so we can decide which way to go.
The online technology that offers the best method does not provide a secure route for collecting sensitive private information such as your nursing license number or social security number. This information is required for awarding contact hours.
There is a combination of other methods requiring more steps for the participant and would delay our first offering while we test it.
Please tell me how important contact hours would be for this work. I was planning to award 2 hours for each offering - 1 hour for reading and assessing the provided studies and 1 hr for participating in the online discussion. Would you prefer to have this sooner without receiving contact hours or delayed with contact hours?
We have established that the registration fee will be $49 for each offering which will include electronic copies of the studies to be discussed, a tool for assessing those studies, plus a 60 minute discussion forum using a webinar format. Sometimes the copyright fees for purchasing one article can be $35 to $50!
The first 3 topics being planned are
Saline vs heparin for locking CVADs - 2 new studies on that issue
Bundles for CVAD care and maintenance (as opposed to insertion bundles) - still deciding on the actual studies
Blood sampling from short peripheral catheters - still deciding on the actual studies
