Hello, I'm looking for standards for Pediatric PICC that discuss over wire PICC exchange when there are several cms of external lenght with the PICC in good position. I have heard if there are more than 4 or 5cms of external catherter length the PICC should be changed out. Please advice, thank you.

See the INS Standards on CVAD exchange. There is nothing stating the length issue you describe but exchange requires a careful assessment to consider risk in patients of all ages. There can be increased risk of infection. This standard has more details. Lynn
