My acute care facility has never had an IV team but we have a very active PICC team. IR is the backup for the PICC
team so we have somewhat of a vascular access program. We are looking at issues related to vascular access
and the possible need for a "IV team". Leadership has decided to put together a PI team and look at different
issues. I was wondering what indicators/ideas you all might suggest we start with to document the many issues/problems  of
vascular access. Â
