I am looking to INS regarding their stance on the use of products such as Pain Ease prior to PIV access. I have been approached by staff and physicians to use this product but my understanding is that this product, or other products used to anesthetise the skin prior to PIV insertion, is not sterile/or aseptic coming out of the container and we must still prep the peripheral site prior to venipuncture. Previously the thought was that prepping the site after the use of this product negated its numbing effects. I am also inquiring if hospitals are using this products are you using it for infants, if so how young.

Aerosol sprays are not labled as sterile, however they must be tested for microbial contamination accourding to the FDA reqjuirements. Look at the labeling on the product you want to use. Does it include IV insertions? The INS SOP includes vapocoolant sprays, the generic name for these sprays, and they are acceptable for use as long as they have a labeled indication.

In regards to the INS SOP, page S94, letter C "Intradermal lidocaine (to be avoided in pregnancy) or bacteriostatic 0.9% sodium chloride", what is the alternative?
Would the vapocoolant spray be a concern for pregnant women?

You would need to read the label and IFU on the vapocoolant spray for any contraindications, however, I not think preganancy is included.

Applying Pain Ease should be the last thing done before doing the procedure. Follow protocol when cleaning the area with an antiseptic, have all equipment ready for the procedure and then apply the Pain Ease immediately before doing the procedure.

Due to low stock, vials of bacteriostatic saline has been limited in my facility and we use it for numbing. Is there a policy on how long we can use the same bottle after it's been opened and can it be used for different patients as long as top of vial has been wiped with alcohol? We're looking for evidence based practice. Thanks.
No, you would be deviating from the standard of care to do either of those. A multidose vial should be discarded no more than 30days after it is opened and is only used fo a single patient. This is based on numerous studies showing cross contamination and subsequent infecious outbreaks when used for multiple patients.
