I'm looking for a form similar to a risk assessment tool where a nurse could "score" a patient's venous access need. Dependent upon the score an order could be obtained for vascular access assessment by our team. Does anyone have such a tool? I looked in the downloads section and didn't find anything.
I've heard that some hospitals with electronic documentation have this in place for admissions from the ECC.
There was a presentation at AVA by Nancy Moreau and Nancy Trick that discussed a Vessel health preservation program and included in this program were forms similar to what you may be looking for. They just completed development of this and are looking at trialing the program at different hospitals to see what works and what doesn't. You may want to reach out to them, and/or get a copy of the program from the AVA website.
