PICC line and billing

I know this topic has been discussed in the past but presenting as an issue for me at this time so wondering if there are any changes -  We would like to offer outpt PICC line insertions in our outpt offices- we are connected to the Acute Care facility so Xrays etc would be easy to obtain.  However, I am not sure how the billing would be done.  I am an MSN and have been placing in the hosptial for some time.  We do not have an IR to place in radiology- very rural area.  If we could place in offices would really help our population from having to go to the OR for placement or drive a long distance.  However, we cannot do this if cannot cover cost of kits etc in some type billing.  Can anyone offer me any information?  Would very much appreciate.  The doctors would send the orders with the patient and there would be a Dr in the bldg in case of emergency

Are you an APRN, such as NP? That is the only type of nurse that can bill directly to payers. You can easily offer insertions in any location but you can only bill directly to insurance companies, Medicare, etc if you are an APRN. You can easily bill the facillity where the patient is located and then this facility bills the payer. That is done all the time. Outpatient is billed differently than inpatient, so this is easily doable. Dr in building is not even necessary as PICCs and midlines and other VADs are frequently placed in locations without an MD within miles. Inserter is responsible for managing any adverse events with or without MD in building.
