Checking to see what other facilities do when infusing Vancomycin in their Peds patients. We currently have a 6 week old that has one failed PIV after Vanco was given. PIV was then placed in the patient's foot. Our policy is not clear for the pediatric population. Has anyone else seen Vancomycin infused through the foot on peds patients?
Is this child walking? Standing? Crawling? Use of foot in any way would mean that is not a good site for any age. How long is prescribed course of VAncomycin? If longer than a week, a PICC would be best.
