Maximum attempts for peripheral IV.

Hello

Does anyone know of studies or guidelines that outline the maximum attempts for peripheral IV (PIV) starts? Most sources mention a limit of 2 attempts per nurse, but how many nurses should be involved? I suggested a maximum of 2 nurses, but leadership is requesting evidence or guidelines to support this. Any guidance would be appreciated.

The INS SOP clearly states

The INS SOP clearly states that only 2 attempts at inserting a short PIVC should be made. If both fail, the patient should be escalated to more skilleful inserter and/or use of US. No patient should ever allow more than this. In fact it happened to me a few weeks ago. I only allowed 2, then required use of US. The US user also failed. All 3 due to absence of good technique and I said no more and cancelled the test.

The INS stated "Restrict PIVC

The INS stated "Restrict PIVC insertion attempts to no more than 2 attempts per clinician." -- It did not say maximum # of clinicians.  We dont have access to ultrasound at this time, and wait times for central lines are quite long (resource constraint).  We also don't have IV team. Many of our cancer patients need to be on schedule for their treatment.  Still advocating hard to get an ultrasound.  I am pushing for maximum 2 clinicians, but again could not find guidelines/studies to support that.

Read page S108 Practice Criteria H where it clearly states after 2 unsuccessful attempts, escalate to a clinician with higher skills ---
