Labeling IV Medication bags with patient information

Our facility is considering not putting patient information on IV bags. Is everyone always putting patient information ( patient sticker)  on their IV fluids with medication, in addition to the date time and initials of the preparer?

I think you need to be more

I think you need to be more specific about what you mean by "patient information". For any fluid prepared with additives by the pharmacy, there is always a complete label with patient name, room number, etc printed by pharmacy. For fluids kept in floor stock or in your ADC those would be opened and hung without pharmacy involvement so may not have a label other than what cmes from the manufacturer.

Thank you Lynn.. To clarify- These are IV medications that the nurse hangs on the unit--for example- an attached vial of antibiotics that is reconstituted for the patient - we are currently labeling with patient label with MR #, BD, admission date, etc.. as well as a medication added label that documents the drug,dose,base solution, date time and initials of the preparer and the start time and flow rate. With are bar code scanning process, alot of this information is captured when they scan the patient . I can seem to find any standard in nursing that requires us to add the patient label with the demographics and there is a wide variety of compliance in our institution. Our other challenge is compliance with wiping out the patient info when we dispose of the IV bag in regular trash to maintain confidentiality. Any thoughts?

See INS SOP 13 Medication

See INS SOP 13 Medication Verification. Are proposed changes to your labeling meeting this standard. Also Standard 57. Is the patient name on the medication label? Are you required to compare the med label to the patient arm band? How do you ensure that the correct medication is being given to the correct patient?
