Anyone in California doing IJ PICC placement?

Our very experienced PICC team is interested in adding this to our practice.  I would like to hear from any other California teams who are doing IJ placements.

A word of caution: I recently reviewed the Ca State Board of Nursing scope of practice for PICC RN and I do not remember seeing anywhere the mention of accessing a central vein by a trained PICC nurse to insert a PICC.

I am not sure if accessing the IJ for insertion of a catheter can still be called a PICC Insertion.

In our facility only IR insert IJ "PICC" to bypass occluded Subclavian veins.

Any other comments about the subject?

The "PI" in PICC stands for peripherally inserted.  Seems to me that an insertion in the IJ is a central line placement, no matter what the inserted line says on it is on the package.  State Board scope of practice would seem to be a big issue, as was mentioned.

Using a PICC is actually currently off label in the language.  Using a CVC or Hohn is not for that vascular site.  As far as performance you have to see how the state board is structured.  If there is an algorithm decision tree with comptency and training one may be able to perform this procedure with a catheter labeled for that vessel site.  At present I know of no RN doing IJ's in California but there are NP's performng this procedure with a labled product.  I would pursue gaining acceptance from medical staff at the facility, develop an educational program and competency training, add preceptorship and approach the state board.  Speak also with the risk management at the hospital.  Yoyu will have to have a comprehensive plan to undertake this procedure and process.
