Incident reporting and peripheral IVs
Recently a suggestion was made at our facility that an incident report be completed for a peripheral IV that is dislodged prematurely either by the pt or staff.  Is it necessary to file an incident report if a peripheral IV is dislodged?  What do other facilities do?  We currently file an incident report for an IV that is removed due to infiltration or phlebitis of a grade 2 or more.  It has not been our practice to do this for a piv that is dislodged accidently or pulled out by the pt.  Does anyone have any thoughts on this?  Thank You!!  Vickie

We recently just switched to the Statlock for PIVs in the inpt areas.  Outpt areas are securing the sites with tape.

Your facility must have a problem with dislodge PIVs and are in need of the data. A manufactured catheter stabilization device is the preferred method for stabilizing all types of catheters including PIV because of the greatly reduced complications associated with these devices.
