Recently a suggestion was made at our facility that an incident report be completed for a peripheral IV that is dislodged prematurely either by the pt or staff. Is it necessary to file an incident report if a peripheral IV is dislodged? What do other facilities do? We currently file an incident report for an IV that is removed due to infiltration or phlebitis of a grade 2 or more. It has not been our practice to do this for a piv that is dislodged accidently or pulled out by the pt. Does anyone have any thoughts on this? Thank You!! VickieÂ

Your facility must have a problem with dislodge PIVs and are in need of the data. A manufactured catheter stabilization device is the preferred method for stabilizing all types of catheters including PIV because of the greatly reduced complications associated with these devices.
