We have recently switched from CLC 2000 cap to interlink injection cap on all CVCs and peripheral IVs. There have been several cases of line disconnections at the connection of  blue threaded lock cannula to interlink extension tubing, resulting in patient blood loss. Has any one experienced this when using these products? I work in  peds heme/onc and IV lines tend to be loaded with three to four lines all hooked to a trifuse attached to each lumen of CVC. Double lumen catheters may have up to six lines attached to lumen. I am not sure if the weight and twisting of the lines in very active children may have contributed to these cases of disconnection. Any thoughts would be appreciated.
