Increase in Phlebitis rate noted after a few hour dwell to 24 hours

Is anyone noting an increase in their phlebitis rate a few hours to 24 hours after placement?    We are concerned and trying to determine the common denominator.  Is it our prep solution, catheter, end cap, ...   The phlebitis is reported to be sporatic on 4-5 units.    Checking to see if others are experiencing a problem also?

 What is common to those areas...are they getting an antibiotic that is highly alkaline or acidic. Did you check securement issues...have you changed to a heavier extension set. Have you changed catheters recently. Is the pharamcy mixing any of the drugs in a different way or have you changed to any pre-mixed or any other IV medication changes.Are all IV nurses starting the lines or does everyone start their own. You have to look at all the factors that contribute to early phlebitis to determine the cause.We see our rate go up when all RNs were starting IVs
