I have been asked to create a customer satisfaction questionnaire regarding the IV Therapy and PICC team. This is for the nurses throughout the hospital to evaluate the services provided to the hospital by the IV / PICC Team. Does anyone have such a form in place that I could look at? Any suggestions would be greatly appreciated...no sense 'reinventing the wheel' if there is already one in existence.
