Your policies re: vein preservation

Would anyone be willing to share their drafts of their policies re: vein preservation for CKD/ESRD?  We are in the process of drafting our own, including the KDOQI 2006 Clinical Guidelines.  We'd like to avoid reinventing the wheel, and I'd love to see what others have drafted.

We're a 450 bed teaching facility with a 24/7 IV Team and ~12 hour PICC placement 7 days/week.

How about some tips then,
How about some tips then, instead of whole drafts?  Does anyone out there practice formal vein preservation for renal patients?  What works for you for criteria - creatinine and GFR? GFR changes?  Did you write the criteria into your PICC order form (PICC requisition)? etc.

There was a roundtable at

There was a roundtable at AVA this year on CKD patients and PICC's. Not sure of the discussion  as I was in another session.  I just looked and the presentor was Randy Cooper, MD from the Southwest Kidney Institute in Phoenix.  It says that vein preservation was one of the topics discussed.  Hope this helps.
