Are there any recommendations as to how often an IV site observation should be documented in the patients medical record in the hospital setting? How often do other hospitals document IV site observations?
Our policy is to document peripheral IV sites every 4h. The exception is a periph IV that is saline locked and not in use for meds (tele precautions ect.). These lines are documented every 8h.

The standard is for each organization to assess their patient populations, types of VADs, types of therapies, etc and establish the interval(s) for their specific patient populations that work for them.
