I have seen many

I have seen many malpositioned lines come out of the OR, some left that way for days, even when subsequent rad reports state the malposition.    It appears that they do not always confirm placment in the OR.   I would find out if that is their practice and never take it for granted.

I would suggest that it should be the responsibility of the IV team to check these rad reports and work to get the catheter tip location confirmed and/or moved into the proper location. In my opinion this should be within the scope of practice for the IV team. I have been in that situation before and gotten these CVCs reinserted. I will freely admit that most physicians did not understand my reasoning but that only provides opportunities to educate them. Lynn
