premedication for blood product administration

I am helping to try to get an order set for blood product administration including using pre-medications, diuretics, and post-transfusion labs. We need to be able to present recommendations from well-known sources to our physician's committee in order to get this implemented. Any help any of you can give is appreciated.

You will need to have access

You will need to have access to the many resources from the American Association of Blood Banks. They write standards, guidelines, and have a textbook. You should be able to locate information in those documents. I don't see how you can create an order set that will cover all needs though as the specific patient's medical diagnosis, history, experience with transfusion such as previous reactions, will dictate what is needed. I have never worked with such an order set for blood but would love to know what you create. Thanks, Lynn

I was curious and had a few minutes before leaving the office. The Technical Manual from the AABB has a very small section on premedications in the chapter on Administration of Blood Components. They include acetominophen, diphenhydramine, meperidine and corticosteroids but emphasize how controversial all of these actually are. So there are no firm recommendations or clear indications for any of these premedications. You should be able to obtain this textbook from your hospital's blood bank. Good luck, Lynn
